Check-in Instructions
In person check-in instructions will be posted to the Turkey Shoot website and will be available during the following times:
For Local Teams ONLY

Thursday, November 20th from 5-9pm at the Hurst Athletic Complex

located at 2104 Precinct Line Road, Hurst, TX 76054
For Out-of-Town teams ONLY

Friday, November 21th from 4pm-9pm at the Hurst Athletic Complex

located at 2104 Precinct Line Road, Hurst, TX 76054

Saturday, November 22th starting at 7am at the Hurst Athletic Complex. All teams must be checked-in one hour before their first game.
The following are the forms you will need upon check-in:

Completed Team Check in Sheet

Official Team Roster (Current Fall 2015 roster) Must be dated and signed by Association Registrar. You must bring a roster EVEN IF you have already sent one to the Tournament Director.

Guest Player Release
Required for any player not listed on your Official Team Roster. Recreational and Competitive teams may have up to 5 guest players. Academy teams may not have any guest players because they can be added directly to the academy roster by your home association.
Must have all required signatures. If a player is not signed with another team, Guest Player Release must be signed by the appropriate Home Association.

Travel Teams: Completed Guest Player Release from your state organization.

Academy teams with US Club will need a Loan Form filled out.

Medical Release Waiver:
Every coach/manager will be required to have a medical release for EVERY player on the game roster with them AT ALL TIMES during the tournament. If a spot-check is done on the field by tournament staff
and they are not produced by the team, the team will forfeit that game.

For U7&U8 Academy - U9 and up ONLY: Game Roster and Misconduct Report MUST be typed; these will be verified at check-in and stamped approved. Your team will NOT be allowed to play without an approved Game Roster during preliminary games AND finals.

Type names of coaches, managers, and all players with a jersey number.

Top portion game information is to be left blank.

You agree that a duplicate of the Game Roster and Misconduct Report with the top portion game information complete will be given to the referee prior to the game start.

Zero Tolerance form to be signed and enforced by team coach or manager. Please review these with your teams prior to the tournament.

Travel Teams – outside North Texas State Soccer Association(NTSSA) must include the following:

You agree that you will have in your possession during each game your travel ID cards (players, coaches, and managers) issued through your team’s home association. Tournament officials will conduct random checks during the tournament.
